Based on the troubleshooting done, seems that the PoE port is not responding this unit should be RMA (return merchandise authorization) in simple words replace it.

Thank you for the response and the information you shared.
Please note, both Access Points need to be upgraded to the same version in order to get them connected on a single Unleashed system.
If you are unable to perform the Unleashed Upgrade online, please perform an offline/local upgrade.
And you can Upgrade to version 200.14.6.1.199 which is recently released.
Please refer to the below video showing the process of Local Upgrade.

It seems like your ICX7150 is running 8090j code. The recommended firmware version for ICX 7150 Switch is SPS08095dufi.bin .
Please perform an upgrade to the code 08095g UFI and upgrade both partitions with the switching code file named SPS08095gufi.bin,
1.Upgrade a Switch ICX-7150-C12P using a USB flash drive
2. Upgrade a Switch ICX7150-C12P via TFTP
3. Upgrade a switch via Web GUI (ICX7150 Switch)

Please perform the below commands.
enable
configure terminal
ip address 10.106.0.10 255.255.0.0
ip default-gateway 10.106.0.1
write memory
exit
After performing the above commands, please share the output of the below command.
enable
show ip address
ping <ip address of the master AP> (Please use IP address of the Master AP here)

Thank you for the response and the information you shared.
The ‘show who’ command output seems to be normal.
Please remove both the Switches from the Unleashed.
Once removed, reboot the Switch once by performing the below commands.
enable
reload
y
And also, please reboot the Access Points as shown in the below screenshot.
Once the Access Points, Switch reboots and comes online please check the Switch status on the unleashed.
Please share the screenshot of the same.

Thank you for the response and the information.
Yes, there will be only One Switch that will appear in the Unleashed that is trying to get in sync with the Unleashed system. Since we were trying to add manually maybe there were 2 Switches got listed previously (One trying on its own and the other one we tried to add manually).
At this point, we should factory reset the RUCKUS ICX-7150-C12P Switch as it is not getting in sync with Unleashed with the other troubleshooting steps we tried.
Please note, there will NOT be any loss of data or configuration. Factory reset will just reset with the default configuration.
Please perform the below commands to factory reset the RUCKUS ICX-7150-C12P Switch. (Also please refer to the below screenshot)
enable
erase system factory-default
y
Once the factory reset is complete, the RUCKUS ICX-7150-C12P Switch will discover the Access Points connected to it and it gets connected to the Unlashed system.
Please make sure the management approval is enabled (as described in the below link).

Based on the troubleshooting done, and you confirm after performing the reset of the Access Point, checking all the connectivity and connections, the failure persists. This Access Point Unit Should be RMA (Return Merchandise Authorization) in simple words replacing it.

If you haven’t tried Accessing Unleashed on a web browser via the IP address of the Access Points, could you please try to find the Master IP address of the Access Point and try to check it on a browser? Please refer to the below guide to find the IP address.
How to find my Ruckus “Devices IP address” using free applications:
You should see the below Unleashed dashboard after finding the IP Address of the AP. Try to log in
Refer to the below screenshot to know your MASTER IP Address on the Web Unleashed system.
Once you find and can log in via the IP Address of the Unleashed Master Access Point, please try the below steps.
1. Make sure you are connected to the RUCKUS Access Point Wi-Fi Network.
2. Click on ‘change login destination’. (Please refer to the below screenshot)
3. Put your Unleashed MASTER Access Point IP Address as ‘Destination IP’ and save it. (Please refer to the below screenshot and 0.0.0.0 is just an example)
4. Now try to log in to Unleashed App using your credentials.

When I complete an IP scan I have two (2) switch manufacturers show up, one with Ruckus Wireless and the other with Brocade. I can login via web browser on the IP for the Rucks Wireless and not the IP for the Brocade (error can't reach this page). I do not show an IP for the APs nor do I see their MAC address listed. Both APs MAC address start with number 9 and nothing in the IP scan has a MAC address starting with number 9.
On the Unleashed app I changed the location to the IP address for the Ruckus Wireless or the Brocade IP address and both give the same error message "Unleashed Login failed". Please be advise my VPN on my phone is turned off while connected to my AP SSID.

Please try to perform the physical factory reset of the access points using the below guide.
NOTE: To perform a factory reset correctly on one of the access points, you must disconnect the rest of your other access points from the "ICX-7150-switch". For example, if you have 3 access points, you should go to the switch and disconnect 2 of the cables to turn off the "2 access points" and only leave 1 connected with which we are going to work the factory reset once you configure the access point and make sure all is good, go ahead and reconnect the other Access Points back.

Before I reseted the AP, I was able to login to the unleash.ruckuswireless.com via my web browser and it only showed one (1) AP connected. I then unplugged the other AP and followed the instructions to reset it. After the reset was completed, I was able to sign into the the Unleashed app only through the unleash.ruckuswireless.com location. I reconnected the second AP to the switch, then waited for over 30 minutes, and still the second AP is not listed in the Unleashed app.
The 5G and 2.4G LEDs on the second AP are not on, the CTL is green blinking "very slow" and the PWR is green blinking "slow".

Hi @needhelp
Based on your description, there are no lights on 2.4 and 5g which means that the radio is not configured.
To configure the AP please try to perform the factory reset on your second AP (which has no lights on 2.4 and 5g) by connecting it to your switch and disconnect the rest of your access points from the "ICX-7150-switch".
NOTE: Make sure you disconnect all other APs on the network and preform the Factory reset to the second AP that remains connected to the ICX-Switch 7150.

Once you are done with the above process please reconnect the other access points back and try to setup in the unleashed using the below guide.
