Replacement Switch - Cant Telnet

I just got my replacement switch and when I try to telnet to it I get:

I go to the web and try to login but none of the combinations work.

How can I fix this?  I am not returning the old one unitl this gets resolved.  I also tried a console cable and that isnt working because username/passwords dont work.  THIS SHOULD HAVE BEEN TESTED BEFORE SENDING OUT!

We are glad to know that you have received the replacement Switch.

May I please know if you have tried plugging the Switch and if the Access points are working?

Also, please share the light status of your Switch.

As it is a replacement Switch, the firmware and the settings will be the latest one.
