Replacement Switch - Cant Telnet

I just got my replacement switch and when I try to telnet to it I get:

I go to the web and try to login but none of the combinations work.

How can I fix this?  I am not returning the old one unitl this gets resolved.  I also tried a console cable and that isnt working because username/passwords dont work.  THIS SHOULD HAVE BEEN TESTED BEFORE SENDING OUT!

Thank you for the information you shared.

As per the case description, I  understand that you are unable to login to CLI through Telnet and showing the  Connection refused error  as the Telnet is not enabled on the Switch.

I would request you to please try to access the switch via the console and use the CLI to perform a password reset.

Once you have successfully reset the password, you can access the Switch CLI.

Please follow the below RUCKUS Lennar Knowledge base self-help article which will help you to console in the ICX-7150-12CP 'Ruckus Switch

Once you have access to CLI using the console cable, please refer to the below video which will help you to reset your password on the Switch.

As I stated I tried going through the console and it still asks for the username/password

I did that and I am still getting connection refused trying to telnet via putty.  What do I need to enable in the switch?

Please refer to the below picture and share a picture of the same, showing the light status and the FULL Switch panel cable connections to the Switch ports

Also, please share a screenshot showing the error while connecting through Telnet.
