Switch icx 7150-c12p is not powering POE, running slow, have to constantly restart.

Switch is an icx 7150-c12p.  at the moment no port is providing POE.  I have to use an injector to have the Ruckus AP working.  I have to restart the switch a lot for maybe a port or 2 to work.  Things are working slow as well.  

I have things connected to 9 of the ports and only 4 are showing up.  Switch says POE is ok but it is not. Temperature is ok.   Please see output from switch below.   Please let me know what we can do to replace.  This switch is broken.

I am not able to replicate the issue to provide a screen shot.  When trying to do copy tftp flash 192.168.1.166 SPS08095gufi.bin primary, I kept getting errors.  It will start but say that the firmware was not compatible for this hardware or something along those lines.   Using the SPS08090kufi.bin seem to work on CLI but I kept following the web GUI steps.   Now I have SPS08090kufi.bin, see below.  Now that I have the update it seems to want to take the other older firmware upgrades.  

I was able to install 08080f but when warm restarted switch was dead.  I waited a while but had to do a hard power reset.  So, to me the better version is 08090k so far.  We will see how this works.  So far so good.  Thank you for your assistance.

Thank you for your response and the provided update.

Based on the commands you shared, it appears that your RUCKUS ICX-7150-C12P Switch is running an outdated non-UFI 08061a version, which is likely causing the issues you are experiencing.

I recommend upgrading the switch to the recommended version. For detailed instructions on the upgrade process, please refer to the following RUCKUS Lennar Knowledge Base article:

Since you have access to the CLI, I suggest following the 2nd upgrade option outlined in the article.

I just warm restarted the switch and took 1 cable outa and connected using an injector to power the device connected to the switch.  Now you can see more ports up.

Well, that outdated firmware maybe an issue, but that doesn't change the fact that multiple ports when I connect POE devices do not power up.  Also, per the output I sent before it says low voltage.  This looks more like a hardware issue.
