Every couple days the RUCKUS device loses power, but if I unplug and plug back in, lights return and everything is on/back to normal. It happened this morning when I woke up and just happened for the first time during the day while using the internet, so it is becoming more frequent.
Started maybe a week or two ago, possibly after a power outage.

As per the case description, I understand that your RUCKUS devices disconnect randomly after power outages.
This issue may happen when your RUCKUS ICX-7150-C12P Switch is running on an older version. To check and assist you with the same please follow the below process and help me with the queries.
Please follow the below steps and help us with the queries,
1. To check this issue, we must identify the ‘current’ IP address assigned by your local router to the switch ICX-7150-C12P
I will leave you a guide on how to perform an IP scanner on your local network to identify the switch IP address very easily.
How to find my Ruckus “Devices IP address” using free applications:
2. Once we identify the IP address of the switch proceed to access remotely to the device “Command Line”
How to remotely access to 'ICX 7150-C12P' or 'access points' "Network devices":
If you can access the Command line should look like this:
ICX7150-C12 Router> OR
ICX7150-C12 Switch>
Please enter the below commands once you log in to CLI and share the output.
Note: Hit Enter to accept each command
enable
show version
show flash

I found the IP by connecting to my ISP Gateway.
192.168.1.103 / ICX7150-C12-Switch
But I can't connect to it using PuTTY or MobaXterm
all username/password combinations do not work

Could you please try to log into the Putty using the Telnet option as described below?
Telnet – Port 23
If you can access the Command line should look like this:
