ICX7150-C12P Ports appear to be failing - RMA

I have an ICX7150-12CP with multiple ports that appear to be failing.  I have a R510 that was plugged into port 1/1/12 and showing a connection speed of only 10Mbps.   When the R510 was removed from ceiling and plugged directly into port 1/1/12 with a known good cable, connection speed would only show 10Mbps and would not connect.   Switch was power cycled and same behavior.  The R510 was moved to port 1/1/3 with same cable and the unit connected normally at 1Gbps. Configuration verified and no speed specific settings on the port.  Please provide RMA of switch.
