ICX7150-C12P Flashes yellow light on power and then dies (Lennar Home)

Hello Team,

We got Ruckus switch and AP as part of Lennar house in 2018. Over the weekend my switch died as a result AP are not powering up because they are set up to PoE (power over ethernet). 

I am new to the community but have read through all the posts and my issue is similar to switch not powering up

Behavior:

1. When powering up ICX7150 , all status lights flash and then everything is off. 

2. I tried reset that did not help.

3. Moved around and plugged into multiple outlets but the result the same.

I did not know if I was supposed to reply to existing tickets are create new one. Please let me know next steps. I have the video of the issue as recommended.

Thank you for sharing the video.

Based on the troubleshooting done, your ICX-7150-C12P Switch is not powering up even after connecting it to working power outlet, and when plugged in, all lights are off. The failure persists. This Switch unit Should be RMA (Return Merchandise Authorization) in simple words replacing it.
