ICX 7150 Won't Write to Flash

I have an ICX 7150 running on the latest SPR UFI code. I am trying to upgrade to the Switch firmware but I am getting an error writing to flash. I have tried from the Web CLI and the console through SSH. Each time, it copies and fails at 25% when writing. I have tried a new download and after several reboots, but no go. I have saved off the support files from the web upgrade.

Ok, i think I found the issue. I didn't need to run the Mount Disk0 command because the drive was already mounted. I am trying to flash the firmware now. 

I attempted the upgrade via USB again and it failed as it previously did.

Greetings!!!

Please try software recovery on your ICX-7150-C12P Switch as described below.

We need to perform a software recovery since it can be that the image flash is corrupted in basic words the unit is not able to boot up properly and need re-install the image to boot up properly.
