POE Failure

I have an ICX 7150-C12P switch in a Lennar Home setup. The PoE controller is failing, causing connected Access Points to lose power. Attempting to modify or save inline power configuration triggers a fatal SIGNUM#11 kernel panic and core dump even after a full erase startup-config factory reset.  Going through all the troubleshooting online has told me that I most likely have a chip failure

If PoE is enabled, then please share these outputs:
