ICX7150-C12P Ports appear to be failing - RMA

I have an ICX7150-12CP with multiple ports that appear to be failing.  I have a R510 that was plugged into port 1/1/12 and showing a connection speed of only 10Mbps.   When the R510 was removed from ceiling and plugged directly into port 1/1/12 with a known good cable, connection speed would only show 10Mbps and would not connect.   Switch was power cycled and same behavior.  The R510 was moved to port 1/1/3 with same cable and the unit connected normally at 1Gbps. Configuration verified and no speed specific settings on the port.  Please provide RMA of switch.

Thank you for the response and the update.

In the output of the show lldp neighbors detail command, I do not see an Access Point connected to port 1/1/12.

Could you please try applying the command below to all switch ports with 'write memory' command and check the results?

enable

configure terminal
interface ethernet 1/1/1 to 1/1/12
speed-duplex 1000-full

write memory
end

Once done please share the output of commands:

show lldp neighbors detail

show inline power

Correct, the port will not establish a link at 1000-full and will only negotiate to 10Mb.  At 10Mb, the port goes on and off line.   The same access point with the same cable has no issue linking on Port1/1/3 with the port hard set or set to auto.  Port1/1/12 is a hardware failure.  Please provide next steps or a replacement.

Please note that, as per policy, we are unable to proceed with a replacement until all required troubleshooting steps have been completed.

1. Kindly share the command outputs after performing the requested steps from my previous response.

2. Additionally, please proceed with a factory default reset of the RUCKUS ICX-7150-C12P switch using the commands outlined below (also refer to the screenshot provided).

3. Have you tried performing a physical factory reset on the switch? If not, please refer to the RUCKUS Lennar Knowledge Base self-help article below for detailed instructions:

After performing the following: 

configure terminal
interface ethernet 1/1/1 to 1/1/12
speed-duplex 1000-full

all connectivity to the switch was lost.   I performed a full factory reset to the switch via the front panel in order to reestablish connectivity to the swtich.
