ICX 7150-C12P-2X1G powers on for one second and won't stay on

I have a Lennar smart home built in 2018. My ICX 7150-C12P will not stay powered on. When I unplug it in the led light blinks once then it immediately powers off. It will not stay on. I tried reset the ICX as well as power off ICX, turn on ICX while pressing the reset button.

Thank you for sharing the requested PICTURES and a VIDEO.

Based on the troubleshooting done, your ICX-7150-C12P Switch is not powering up, and when plugged in, it flashes an amber light and then goes dark. And you confirm that the Switch is plugged into a well-known outlet/power source. However, the failure persists. This Switch unit Should be RMA (Return Merchandise Authorization) in simple words replacing it.

The lamp (white light) is plugged into the same outlet to demonstrate the outlet has power.
