Switch ICX7150-12P no lights

I have an ICX-7150-12p. The switch started to randomly power itself off. I swapped power ports and the same condition occurred. I swapped cables and the device is dead with no indication lights. Help.

I have a dead switch that came with my Lennar home. How to get it replaced? I have swapped cables and outlet, no go.
