I am trying to update my firmware for a malfunctioning Lennar/Amazon ICX-7150-C12P. None of the default passwords mentioned in other posts work to access the router. I tried booting in "no password" mode several times, and the router still asks for a password.

Thank you for sharing the requested information.
Based on the troubleshooting done, you confirmed, and we saw that after the reset, booting up the Switch, and trying to get the console access, the failure persists. This Switch unit Should be RMA (Return Merchandise Authorization) in simple words replacing it.

Yes, I have tried logging in using all of the default passwords. I also tried the “no password” at bootup. The device still asks me for a password.
