ICX7150 does not provide power to PoE Ports

Hi, I have a ICX7150-C12P switch serial #FEK3852Q196 that all of the sudden does not seem provide power to any of the PoE ports, so the two access points we have connected to the PoE ports do not turn on.

I tried power cycle and reset button a few times, but still same issue. The status lights on the switch seem to turn on correctly. However, the lights/indicators on the PoE ports and access points never turn on. 

Please advise,

Thank you for the response.

As you mentioned you are blank screen on the putty, could you please share a picture of the Switch and Laptop/PC showing the cables connections?

And please note, we do not use Telnet to establish the connection, it is a Serial connection as shown in the below screenshot. Please make sure you are using the correct COM port from the device manager.

Please help us with pictures of your ICX-7150-C12P Switch and Laptop/PC showing the cable connections.

Thank you for the response and the information you shared.

As I can see from the picture, you have only used USB to type C cable. Also there must be a ethernet cable connected to the Switch management port and the Laptop/PC. (Please refer to the below screenshot)

Please use ethernet cable (both ends RJ45 plugs) and USB to type C cable both together as described in steps 2 & 3 in the guide link. Request you follow the same and perform the software recovery process once again. And please share the screenshots of the error (if any).

Hi Imran, I was able to update the switch per your advice and I waa able to get it out of the Boot mode using the SPS08090k.bin firmware. The switch seems to be now working fine. However, I tried to update to the recommended 08095g UFI code with the file named SPS08095gufi.bin and keep getting the error below, please advise:
