Having some issues on a couple of ICX7150-24-POE switches with the POE Severe Errors. POE devices will no longer have their requests honored. There was 11 POE devices on ports 1/1/1 to 1/1/11. Port 1/1/13 is reporting Oper State Non-PD and the Fault Error is: Voltage applied from ext src. Nothing is plugged into that port.
I remove the 11 POE devices, only the fiber sfp uplink is left plugged in. "No inline power" all 24 ports on the switch. Reboot switch. Enable inline power to all 24 ports. "show inline power" port 13 is already displaying the above error message. Plug in a POE device in to 1/1/1. No power. Check logging have the POE Severe Error show up now for port 1/1/1. The odd thing that happens though is if I plug a POE device to port 1/1/13 it powers up and is fully reachable. I do "show inline power" and the 1/1/13 line is still the same as before Admin On/Oper Non-PD, showing 0 Consumed/0 Allocated and the Voltage Fault Error.
I upgraded the switch image from SPS08070db to SPS08095s (UFI). POE firmware is currently at 02.1.8 Build 004. The only thing I havent had a chance to do is to completely factory default the switch. Anything else I should try for troubleshooting purposes.

This may be a hardware issue, but I would request you open a case (see my signature) to get official support on this one.
