We were seeing issues where it appeared that our ICX7450-48p units would have the POE modules fail at random in the field but on recovery would seem fine. After a lot of troubleshooting we found that certain model Dell Optiplex with Intel(R) Ethernet Connection (17) I219-LM cards will cause the switch POE to fault when plugged into the network. We have seen this with multiple switches at multiple locations and they all recovery following a reboot if the Dell is disconnected.
The switches all had the same POE hardware...

The product there denotes the motherboard model. This specific Dell eventually had a board issue and an RMA. Afterward the model changed to rev A01 and that unit no longer caused this issue....
We have seen this issue with certain Dell models purchased as recently as a month ago.

I have a question for you here :
Have you tried any troubleshooting steps? For example, enable-disable the PoE, perform a software upgrade.
Did you have a power outage?
The switch was connected to a UPS?
Did you test it on several ports? What was the result?
Could you please share the below logs
sh inline power
sh inline power detail
sh inter ether x/x/x -interfaces affected by the PoE
show inline power emesg
show chassis

We're not using any optics or SFP in these scenarios. This is happening when a specific model of Dell Optiplex with the Intel ethernet card is plugged in using cat5/cat6 to an ICX7450 with the listed POE hardware module. The POE faults until the Dell PC has the Rj45 unplugged.
We've had roughly 2 dozen Optiplex's exhibit this behavior with a dozen ICX7450s in deployment. At this point we're not really looking for a resolution because we know there likely won't be one as this seems to be a hardware compat problem between vendors, but want people to know this happens and why if they encounter a mysterious POE outage after buying a Dell PC.
