ICX 7150-12P switch failing to load kernal during boot

Our Ruckus ICX 7150-12P switch is not working (has never worked in the past). I finally tried to access it via the command port to see if there is a settings issue. It seems like the boot loader never successfully loaded the boot image. I connected to the switch via PuTTY using serial connection. Any help will be appreciated. Following is the output of a reset command that I ran:
