We were having some issues with certain phones/AP's losing connection to the switch. Plugging them into another switch in the stack solved the issue. I reloaded the 3-switch stack last night and since then, POE hasn't worked on the faulty switch(#2). POE is administratively on, but when I plug in a PD, it's just not detected and never enables the port. The 6 POE hardware instances show 2 as Recovered and the rest Good. This feels to me like I need to re-enable POE This is an ICX 7250 running SPS08095g.bin. Below are the output of 1. show inline power, 2. show inline power detail, 3. show chassis

Thank you for reaching out, considering the behavior of unit 2 when connecting a PD and also this entries:

I would suggest opening a ticket with the Wired TAC team as it would likely end up in an RMA, click here to open a new case.
