I'm trying to configure a QoS policy for our hosted telephony and I'm trying to create an IP based UDA with a port range of 9000-10999. When I try to save the settings with a range defined I receive the error "Port is not a valid port number or port range.". Does anyone know what the correct format is for a port range?

That field refers to these kind of port numbers.
I think it won't accept a range. About the numbers you tried first 9000-10999 what are they for?
