Thank you for sharing the requested information on an private chat.
Based on the troubleshooting done, your ICX-7150-C12P Switch is not working with console cable to access CLI and not getting IP address assigned from the ISP (Internet Service Provider's) Router. Checked with multiple troubleshooting steps. However, the failure persists. This Switch unit Should be RMA (Return Merchandise Authorization) in simple words replacing it.

Based on the information provided, it appears that you are encountering Routing code issues with your RUCKUS ICX-7150-C12P Switch and are unable to find the IP address to fix the same.
We apologize for any inconvenience this has caused and are committed to assisting you promptly.
If you are unable to get the IP address of the Switch, please try getting console access to the Switch and help me with the command outputs I shared previously.
Here is a guide on how to access the Switch via console (cable access between your PC to the ICX 7150-C12P) in this case we need a USB type C to USB standard OR Console cable Rj45 to USB (Any one cable- please refer the guide)

I connected to the switch via USB. I'm unable to login with any user name and password. Even after factory resetting the switch I cannot login with the factory default username and password.

While you have the laptop connected to the Switch with a console USB cable and the CLI screen up it asking a username/password, reboot (unplug and plug from the power outlet), and follow the video below.

Once you have successfully recovered the switch password, use the following commands to either modify the existing password or create a new username and password:
enable
configure terminal
device(config)# username super password pass
In this example, the username is ‘super’ and the password is ‘pass’.
To ensure proper authentication, execute the following commands:
device(config)# crypto key generate rsa
device(config)# aaa authentication login default local
device(config)# aaa authentication web-server default local

Thank you for the instructions, Imran. I was able to get to the boot menu, enter 'no password' and then reboot the device. However, I'm still being prompted for login.

I was able to run the factory set-default command successfully. The switch booted both images without error. However, I’m still prompted for login. What steps should I be performing after the factory set-default step?

Yes, after the Switch reset, there will be a default password.
Could you please once again try with the Switch default credentials as mentioned below?
Username: super
Password: sp-admin
If the above credentials don’t work, you can try the default credential combinations mentioned below in the screenshot.

Additionally, you can reset your Switch username/password if you want, use the following commands to either modify the existing password or create a new username and password: (Please note them for future reference)

After these commands you can use new reset username/password
When I login with user name and password and enter enable I get the following message

The return message is: Key already exists. Please zeroize it.
I was also able to go through the following steps; however, when I run Show Version it still shows its running the routing code.
To summarize, I now can login, change password, boot from secondary, flash secondary but still appears to be running routing code. I'm unable to login to the switch via the app only via USP to console. I still get ‘no password assigned error’ after entering ‘enable’ after successful login.
Please reset your username password once again using the below process.

Please download the Putty Application and try to log in to the CLI by SSH option using your new username and password.

I performed the following steps

The username and password I set worked as I logged out and logged back in using the new username and password. However, when I rebooted the switch the username and password I set no longer works. I also tried other username and password combinations and nothing work (referencing previous screen shot of possible username/pwd combos).
I went back to the instructions above on recovering the switch username/password and follow the instructions again in the video: enter 'no password' and then boot. once the switch reboots I'm still prompted for a username and password.
Upon reboot I also continue to see errors in the boot process and TFTP timeouts (screenshots provided earlier). I also see this error when rebooting regarding could not boot from primary
Is there any other way to get support other than messaging back and forth in this forum. This is not a helpful avenue for such in-depth support. In absence of any other support avenue. What steps should I try next if the password recovery method is no longer working?

Please reboot the switch by unplugging it from power and then plugging it back in, ensuring that the Console CLI is connected as we did during credential reset process. Once the switch displays its version, you will find the serial number just below it; please refer to the screenshot below for guidance.
