Simple external login page with NBI (use Extrenal Radius) for vSZ

Hi everyone,
I am using vSZ. I am trying to build external login page authen by Radius Server. I refered this link but it is sample without Radius Server.

Can anybody help me a simple form login page in the web server that it can integrate with NBI?
