ICX Switch Failure

The ICX 7150 switch installed in my home has failed POST and remains stuck on a solid orange SYST light. I performed a hard factory reset with all network cables disconnected, but the unit is unable to load its primary OS image. Multiple attempts have not resolved the issue.
