Support Case Summary for ICX7150-C12-POE – PoE Fatal Vmain (48V) Fault

Switch Model and Serial

  • Model: Ruckus ICX7150-C12-POE

  • Serial #: FEK3249N11X

Current Firmware and Boot-Monitor

  • Switch OS Version: 08.0.80fT211, previously 08.0.90kT211

  • Boot-Monitor Version (current): 10.1.14T225 (mnz10114), previously 10.1.15T225 (mnz10115)

  • Recent upgrade attempts: Updated both system image and boot-monitor via TFTP per official Ruckus documentation.​

Troubleshooting Steps Performed

  • Performed full factory reset and reload, confirmed startup with factory defaults.

  • Upgraded firmware and boot-monitor to latest available per Ruckus support downloads.​

  • Attempted configuration changes, checked running config, and verified enable/admin credentials.

  • Power-cycled unit multiple times, including long power-off for several hours.

  • Attempted to initialize PoE by plugging known-good Ruckus R510 APs, using original and alternate Ethernet cables.

  • Ran extensive CLI and console commands for system status and PoE (“show version,” “show running-config,” “show boot-monitor,” “show inline power”).

  • Observed boot logs and live console output during normal and TFTP recovery/reflash cycles.

  • Confirmed power supply detection, normal system initialization, and correct PoE module reporting (before error manifests).

PoE Fatal: Vmain (48V) fault has forced all ports on slot 0 to lose power. U1-MSG: PoE Severe Error: PSU voltage is lower than min limit. Port 1/1/3 powered down. RMA the PSU. PoE: Power disabled on port 1/1/3 because of PSU voltage drop. PoE Fatal: Vmain (48V) fault has forced all ports on slot 0 to lose power. (repeated for all PoE ports 1/1/1 through 1/1/12)
  • No PoE device is able to receive power even briefly. Oper state always “Off”; power allocation = 0. No cable/AP/circuit resolves behavior.

Summary and Request

All recommended troubleshooting steps (factory reset, power cycle, firmware and boot-monitor update, config/CLI analysis, swap cables/APs) have been exhausted.
Persistent and recurrent “PoE Fatal: Vmain (48V) fault” and “PSU voltage is lower than min limit. RMA the PSU” clearly indicate a hardware failure in the PoE subsystem or the switch's internal PSU.
Please advise on replacement or RMA procedure as suggested in the manufacturer error logs.
