Are Ruckus One Trials still available?

The Ruckus One Trial page says to "fill out the form below" for a trial, but there is no form.

(I tried creating a My Ruckus account, in case that was necessary, but it hasn't helped).

There's a form for me on the right side of the screen.

Thanks. That pointed me in the right direction. My company proxy server & my home firewall's pfBlockerNG both block that content. If I use my phone hotspot then I can see the form.

MS Edge Tracking protection also blocks this form.
