Ruckus switch not working

My Ruckus switch (7150-C12P) is not working. I tried to power off and on by disconnecting the power cord, it briefly flashes amber then no lights. I tried to do the factory reset button as well, same result with quick flash amber lights then no lights. Based on other searches in this forum, it looks like an RMA is required. How do I initiate that process?
